Peripheral Blood CD34+ cell enumeration as a predictor of apheresis yield - an analysis of over 1000 collections.

Abstract

The role of the peripheral blood (PB) CD34+ cell count for predicting the CD34+ cell yield in hematopoietic progenitor cells apheresis (HPC-A) collections is well established. However, unexpectedly poor CD34+ cell yields are sometimes obtained. To determine the effect, if any, that a range of factors had on the ability of the PB CD34(+) count to predict collection CD34+ cells, we performed a retrospective analysis on consecutive HPC-A collections during 2004 to 2008. Factors investigated included mobilization regimen, PB WCC, patient body weight and disease. After exclusion of collections where an apheresis complication occurred, 1225 PB CD34+ cell results, with corresponding collection CD34+ cell results, from 458 patients, were analyzed. Although differences in the median PB and collection CD34+ cell were observed between chemotherapy+G-CSF vs. G-CSF alone mobilized collections, the predictability of the PB CD34+ cell count for the collection CD34+ cell yield remained similar. Whilst poorer collection efficiencies were observed in the Myelodysplastic (MDS)/Myeloproliferative Disorder (MPD) diagnostic subgroup, our study confirms that PB CD34+ cell analysis for prediction of HPC-A CD34+ cell yield remains a powerful and irreplaceable tool.
