Abstract
Rheumatoid arthritis (RA) patients are at increased risk of lymphoma. However the risk of lymphoma secondary to anti-tumor necrosis factor (anti-TNF) therapy in RA patients is currently debatable. Primary breast lymphoma is a rare entity and to our knowledge this is the first case of primary breast lymphoma described in RA patient receiving anti-TNF therapy. Our case is quite unusual and interesting because the lymphoma presented initially as breast abscess and the patient had prior history of infiltrating lobular carcinoma of the contra lateral breast. The lymphoma metastasized to the lungs and retroperitoneal lymph nodes. She received 8 cycles of R-CHOP [(Rituximab, Cyclophosphamide, Hydoxydaunorubicin (doxorubicin), Oncovin (vincristine), Prednisolone] chemotherapy. She did very well and both the lymphoma and the rheumatoid arthritis went into remission. We would like to point that given the increased risk of unusual lymphoma in RA patients receiving anti-TNF therapy, they should undergo routine gynecological examination.
