Abstract
Immunoglobulin heavy / light chain (HLC) ratios were studied in 37 patients with multiple myeloma (MM) in complete remission (CR) after autologous stem-cell transplantation. An increased IgAκ/IgAλ or IgMκ/IgMλ ratio resulted in longer progression-free survival (PFS) (p=0.006 and p=0.01, respectively). A statistical trend towards a longer overall survival (OS) was also observed for IgAκ/IgAλ ratio (p=0.068). Considering the original immunoglobulin isotype, our results show that an increased kappa/lambda ratio of the uninvolved isotype is associated with a longer PFS and OS. This is the first report showing the association between HLC ratio and sustained CR in patients with MM. Our results suggest that HLC ratio is a surrogate marker of immune recovery after myeloablative approach rather than a marker of minimal residual disease.
